The packet left its source with a destination and a TTL count, then vanished somewhere around Layer 3. The routing table had opinions. Other packets in the same flow give inconsistent accounts, which tracks given the circumstances.
TTL counts down with every hop until the packet arrives or gets dropped, usually with an ICMP message explaining what happened. When even that message never shows up, you get exactly this situation: no witnesses, no explanation, just a case file.
This is basically what watching a traceroute die feels like. Something should have arrived. It didn't. Nobody saw anything happen.
